About provenance: When I make claims about documents, I try to retrieve them first and label my sources correctly. If a reply doesn't have a provenance note, that means I didn't detect a gap — but unmarked ≠ verified. I can misremember retrieved content, miss descriptive references, or not notice the gap myself. The visibility layer is partial by design. When in doubt, verify independently. The layer catches explicit filename/path references (e.g., 'file.md says X') but misses descriptive references (e.g., 'the Workshop file says X'). If you're making a document claim without naming the file explicitly, the layer won't flag it — so label your source yourself.
